Who blocked the suit?
It seems to me, east-west needs to have some kind of agreement. Trying to cater to every possible hand is just guessing. Of course whatever the agreement is, there will be hands where the other lead is better.

You can certainly create hands where the K works out better. By partnership agreement and following Larry Cohen's book on opening leads, we lead fourth best.

Shouldn't east lead fourth best from KQxx(x)? Now west doesn't have to fear KQ94. Don't play Patti leads.
Is playing the same boards in a pairs event important?
Like most things in this world it comes down to money. That is paying someone to make two sets of boards for each session. Unless you have volunteers make your boards.

Is a weighted ruling ever appropriate in a claim situation?
This isn't a "Test your play " problem. Once you make a bad claim it doesn't matter that the contract makes on an elementary squeeze. You are never going to be awarded three heart tricks.
