You Be The Judge - hand from Wernher Pairs

I thought this hand was of some interest since there are several possible decisions for N/S to make... please feel free to comment about any or all of the specific actions.  N/S playing something of a Polish club style, opening bid style "normal" (not ultralight, but certainly not Roth/Stone either) with 5 card majors and strong NT.

actual table result was -690 (cold for 5, lead and subsequent defense was irrelevant).  Many pairs were -650 defending 5 doubled (which E/W were very likely to bid if N/S bid to 4 or 5 of a minor)

100% blame to North, should have opened 1, or bid 4 over 4
more blame to North
about equal blame
more blame to South, should have doubled 2, or bid something over the double (and if so... 4? or 4NT?)
100% blame to South
Nobody did anything terrible, just an unlucky board (the usual "preempts work", etc.)
