When should you NOT make a Jacoby Transfer when replying to a (weakish) 1NT?

Jacoby Transfers (2 -> 2 and 2 -> 2) have been around a long time and are one of the first conventions a beginner learns after Stayman and Blackwood.

Nevertheless there must be times when the bid is not the optimum strategy. Not having access to Bridge Simulation Software, I would like your thoughts on when a Jacoby bid should NOT be made (as a function of suit quality, distribution, and outside strength) with a 5-card major.

(The weak NT is 12-15 and promises strength in at least 3 suits).
