Here you reach 4 after a straightforward Jacoby sequence. 1-2NT-4. West leads a trump.

For hand one duck the opening lead, win the second heart and draw trumps (they split 2-2) then strip off the clubas and play a third heart. (EDITED TEXT)

East began with the doubleton heart king so West will be thrown in to play diamonds for you. The finesse was off so this makes the overtrick.

For hand two draw trump and play the club ace and duck a club. Ruff the club return and exit with a diamond to the eight. the best the defenders can do is return a diamond but you win and play a third diamond. Even if East can win to play a heart you can run it to the nine -- this line succeeds unless both the Q and 10 are offside.

(taking the heart finesse yourself turns a 75% line into a 50% line).
