It's the last round of the final day Swiss at our local regional here in the DC area.  Your team has done well in a strong, good-sized field and is in 2nd place, only 3 VP's out of first.  You are facing a very good team (though not the one ahead of you since you've already played them) and here is one hand:

N/S play a forcing-but-not-guaranteed-to-be-strong club (somewhat Polish style with 1 being opened on all balanced hands w/o a 5 card major) with strong NT, opening bid style is not Roth-Stone, but not "open any 11 count" either.  4th chair 2M opening bids are "standard", i.e. usually about 10-12 (maybe a bad 13?) with 6+ suit.

The 4 game is basically on the heart hook, though there are slight chances for a diamond ruff.  The hook was on, and game was bid at the other table so we lost 6.  Please assess blame, if any:

100% North (should have opened the bidding, should have invited over 2, etc.)
mostly North
about equal blame
mostly South (should have opened 1 instead of 2, etc.)
100% South
neither North or South is to blame, both made reasonable actions and got unlucky
