West leads the C9, East overtakes and plays back a top club won by North. Declarer plays a spade to the SQ which wins (West ducks but not in tempo). Declarer cashes four hearts and the spade ace to arrive at the following end position:

Declarer exits with a spade which West must win or he will be endplayed from his DA. West's spade return squeezes East down to DQ9 and CQ. Now North exits with the last club endplaying East.

(In reality, that is not how the play went at the table. But it was the way it was replayed in my head over and over at 3am as I lay in bed after the medicine I had taken to fall asleep wore off.)
